A plain-language explanation of what this website collects, why, and what happens to the details you send through the request form.
Last updatedOctober 11, 2026
Key takeaways
- The request form collects only your name, email, optional phone, preferred contact method, who the appointment is for and an optional short message.
- Form submissions are emailed to the practice and are not stored in the website database.
- Please do not share sensitive health details in the form; the website and form are not a secure clinical channel.
- The site uses self-hosted fonts and scripts, no third-party analytics or font trackers at this time, and cookies only for logged-in administrators.
- This is not a HIPAA Notice of Privacy Practices; clients receive that notice separately when care begins.
Reaching out to a therapist can feel exposed, even before the first conversation. You should know exactly what happens to the few details you share here. This notice explains, in plain language, what the Your Present Self website collects, why, and the choices you have. It applies to this website only.
This is not a Notice of Privacy Practices
This page describes how the website handles information. It is not the practice’s HIPAA Notice of Privacy Practices, which explains how health information is used and protected once you become a client. Clients receive the practice’s Notice of Privacy Practices separately when care begins. We do not describe this website or its request form as HIPAA compliant, which is why we ask you not to send health details through it.
What information do we collect?
When you use the request form
Our appointment request form asks for:
- your name
- your email address
- a phone number (optional)
- your preferred contact method
- who the appointment is for (for example, you, a couple or a family)
- a short message (optional)
Please do not include sensitive health details, such as symptoms, diagnoses, medications or what is happening in your relationship. A brief note about what you are looking for is enough. Those conversations belong in a confidential setting once care begins.
When you browse the site
We do not use third-party analytics or third-party font services at this time, and our fonts and scripts are hosted on our own site. The site uses your browser’s session storage to remember small interface choices, such as whether a menu is open; this stays in your browser and clears when you close the tab. Standard WordPress cookies are set only for administrators who log in to manage the site. Like most websites, our hosting provider, Hostinger, may automatically record basic technical information, such as IP addresses and the pages requested, to keep the site running and secure.
Why do we collect it?
We use form details only to reply to your request, to talk about whether working together may be a good fit and to arrange a first conversation. We do not use them for advertising, and we do not add you to a mailing list.
Where does it go, and how long is it kept?
Form submissions are sent to the practice by email. They are not stored in the website database. Emails are kept only as long as reasonably needed to respond to you and to meet the practice’s professional record-keeping obligations. If you become a client, information you share during care is handled under the practice’s Notice of Privacy Practices.
Do we sell or share your information?
No. We do not sell personal information, and we do not share it with advertisers or data brokers. Information may be handled by the service providers that make the site and email work (such as our web host), or disclosed when the law requires it.
Children and teens
This website is intended for adults. If an appointment is for a child or teen, a parent or guardian should submit the request. We do not knowingly collect personal information online from children under 13. If you believe a child has sent us information, please let us know and we will delete it.
Michigan residents and your choices
Whether you live in Michigan or elsewhere, you can ask us what information we have from your form submission, ask us to correct it or ask us to delete it, subject to any professional record-keeping obligations. You can also choose not to use the form at all; using the website does not require you to share anything.
How to contact us about privacy
Send a privacy question or request through the request form and begin your message with “Privacy”. We will reply using the contact method you choose. For how we handle public provider records and correction requests, see our editorial policy.
Changes to this notice
If we change how the site handles information, for example by adding an analytics tool, we will update this page before or when the change takes effect and revise the date shown on this page.
Common questions
Is the request form secure for health information?
The form is meant for simple contact details and a short note. It is not a secure clinical channel, and we do not describe it as HIPAA compliant. Please do not include symptoms, diagnoses or other health details. Those conversations happen in a confidential setting once care begins.
Does this website track me with cookies or analytics?
Not at this time. The site uses self-hosted fonts and scripts and no third-party analytics or font trackers. Session storage remembers small interface choices in your browser, and WordPress cookies are set only for administrators who log in. Our web host may record basic technical logs to keep the site secure.
Do you sell my information?
No. We do not sell personal information or share it with advertisers or data brokers. Your form details are used only to respond to your request and arrange a possible first conversation.
How is this different from a Notice of Privacy Practices?
This page covers the website only. A Notice of Privacy Practices explains how a health care provider uses and protects health information. Clients receive the practice's Notice of Privacy Practices separately when care begins.
